DETAILED STEPS OR PROCEDURE TO ACCESS THE TREATMENT WITH MGUT MUTAMBA

The 8 essential steps and procedural to access the MGU Mutamba treatment for a patient or applicant are the following, which are detailed below:

1. Send an electronic email to Call Center of the Mutamba Project

( supplyarea.mutambaproject@gmx.com ) formally requesting access to treatment.

You can skip directly the Step 1, jumping directly to Step 2.

2. Complete the Registration Form to Treatment Access that Call Center will refer the patient or applicant, or directly access this form via the link posted on the following page:

Complete carefully and fill in all the form fields clearly and properly.  Once completed must be sent automatically by the system.

This step is important to clearly specify the destination address where the treatment is sent.  The patient or applicant shall report any change in the destination address to the  Supply Area (Supply Area or Call Center).

3. The Call Center will answer the patient or applicant with information regarding treatment and payment procedures or additional information, they also are published and detailed in the following page:

4. This procedural step is the most important.  Once the patient or applicant knows the amount of payment for treatment and payment methods available, the patient or applicant must come into direct and previous contact with the Head of Deliveries Area or person responsible or coordinator in charge of it, by email, direct phone or  (WhatsApp) directly, that before the respective payment.  This is to ensure proper amount for treatments or payment and if the treatment shipment can be sent to the location of the patient or applicant.

5. Once done the previous contact and the necessary confirmation of shipping cost by Supply Area , the patient or applicant may pay or normal transaction accessing treatment. It is recommended that the patient or applicant Not Do this step but rather has completed sucessfully the steps 2, 3 and 4 previously without problems.

6. Once the transaction is conducted , the Supply Area will proceed to :

- Set the probable date of delivery of treatment to the patient or applicant submitting this information by email.

- Communicate later the patient or applicant about the shipping treatment performed, the date indicated or marked.

- Provide the patient or applicant with a Guide Number or tracking number of your package treatment.  If the patient does not receive for any reason, must formally claim or demand it by electronic email or by telephone to the Area Supply or Call Center.

7. The patient or applicant shall perform continuous monitoring of treatment by your package tracking tools or links provided by Call Center or perform direct consultation to Call Center attaching the tracking number.

8. Once the package reaches the specified treatment, to the hands of the patient or applicant destination, the patient or applicant must inform the Area Supply and Call Center of the successful receipt, this to close the circuit shipping treatment.

OBSERVATION IMPORTANT.-

If the treatment package does not reach its destination specified by: damage to it, alterations, loss, theft, customs reasons, due to the delivery time of more than 15 days, arrival shabby treatment.  The project ensures forwarding treatment without cost or charge to the patient or applicant.

The Project is not responsible if in step 2, the patient or applicant wrong destination address or the address clearly confirm before shipment for security reasons.
